Apple TV app 'Continue Watching' frozen in time: how to fix?

On 3 separate devices (MBP on MacOS 15.1.1; iPad & iPhone on iOS 18.1.1), my Apple TV

continue watching queue has been frozen in time as of a couple of months ago, with the same sequence of AppleTV programs *and* the same placeholder suggesting I start the same episode at the same place (e.g., S2 Ep3 of Pachinko, at 53 minutes left). Finishing that episode with fast forwarding to get to where I was in the next, and the next, and even completing the series did not trigger any update to the queue.


All the programs shown in the queue currently are Apple programs only on AppleTV; at some point during the month when I had an active subscription to Hulu (not a subscription through Apple nor watched on that app), a couple of Hulu shows also got locked in to the 'Continue Watching' in a similar fashion, as though I was also stuck on S1, Ep8, 22 minutes into whatever it was on Hulu. When my Hulu subscription ended, the Hulu shows dropped off the 'Continue Watching' (still stuck at that same moment in time)--this is the only change in the 'Continue Watching' queue that I'm sure of since the moment things became frozen.


And no other devices are involved--I am watching only on these devices, there is no physical TV of any sort.


I think this frozen moment was from before I updated MacOS to Sequoia and to iOS18 on both iPad and iPhone (iOS updated within a day of release; I did delay MacOS update from 14.x.x until 15.1 was available).


I believe this has survived deleting and redownloading the app at least once, but I think I only did that on one device at a time (likely the iPad as my primary streaming video device). I also signed out of my AppleID, but again, may have only done that on the iPad.
