For the Activation Lock, you use the information that has already been provided in the link. You will need to send in your original purchase receipt showing the Serial or IMEI number of the device. In most cases this will have to be the receipt from Apple or Authorized Apple Seller, as they do not get involved in any third party sales. The reason for that is even I could create a receipt that would mean absolutely nothing.

Without access to the account it is locked to or the documentation required to remove the Lock, then the device will simply remain unusable. Many people who have had their devices stolen are very thankful for these security measures.

So I can't unlock it anymore?

Just the two options that have been given. You have not explained why you are unable to gain access to your account as that is the most common way the Activation Lock is removed and does not require any documentation. Since it is your phone, you must know the Apple Account that was used to set up your device, so you should be able to use Account Recovery to reset your password.

I have lost my Apple account ID password and Apple name.

To start the Account Recovery process you must know the email address used for your Apple Account. Since you do not know that either, you will not be able to gain access to the device. There is nothing left that can be done and it does not appear we have all the information on how you acquired the device or how it became locked with an Apple Account that you have no idea what it is.
