Volume goes to max when I connect headphones via Bluetooth

The title text pretty much says it.

Playing music, watching a video, streaming something, volume is around midway to three-quarters up - then I connect headphones via Bluetooth and when the connection is made, the headphones blast at 100% volume.

I have several different brands of headphones, and this happens with all of them.
