The keyboard will display like the keyboard below only in certain apps. In which apps were you able to see this keyboard?

Seeing the numbers on the top row, accompanied with symbols below is normal, and is how the keyboard was designed by Apple. Only for certain activities, such as changing your passcode, will you see a solely numerical keyboard.

Can see on iPhone 15 but not able to see on iPhone 16. Both are on same OS version. Tried adding and deleting multiple keyboards. Tried G board as well."

Yes, the numerical keyboard will pop up when entering a contact number, because a contact number is naturally comprised of solely numbers.
