If you have a problem with the app, then you should contact the developer directly for a response. The App Store has a link to Support for the App if you are unable to find any contact information. Only the developer can update the app to resolve a bug or something that does not work correctly.
Reporting an app to Apple is for when it violates the Submission Guidelines and also bugs. The submission guidelines can be found here and this is where Apple will notify the developer to give them an opportunity to update the app so it will be compliant with their standards and resolve problems that have been reported by users. If there are many reports and the developer is not responsive, then Apple has the right to remove the app completely from the App Store. There is no information provided to you about any communication that Apple has with the Developer.

Nope. If Apple receives multiple complaints about an app that violates the Submission Guidelines for allowing apps in the App Store, then the developer may have an opportunity to update the app or Apple may simply remove the app from the store. You will receive no further communication about this process.
Each situation is different and personally for me I would consider Facebook a scam just from the false information that is seen from many of the posts. I can report the app as a scam, but it is not likely to result in any action as there is not a violation of any of the Submission Guidelines. Also what I consider a scam, other users may have a different opinion.
In addition to reporting the app to Apple, you can also write a review for the app in the App Store. This will let others who are looking to download the app aware of your experience and you may even see a response by the Developer.
