How can I set up the monthly payments and not to the one big charge at the beginning

If you mean interest-free monthly installment payments for purchases from the Apple Store (not the App Store) using an Apple Card credit card, see: Apple Card - Monthly Installments - Apple

and Products you can buy from Apple with Apple Card Monthly Installments - Apple Support

Be aware that you need to use an Apple Card and need to specify Monthly Installments during the checkout.
