I have an Apple Watch 9 and wanted to use the modular watch face. I would like to add the heart rate complication to the face, but the option is not available to add for selection in the iPhone app.

So after clicking "Add" to Download watch face for the Modular Face to your PAIRED Watch Apple Watch did you for example click on one of the Complications positions (lets say Bottom) to change the current complication for the Heart Rate complication? Doing this will take you into the WHOLE list of available complications for you to select. Just find the Heart Rate complication from the list and check✔️ it to change (Bottom) it to the Heart Rate on the Watch Face.

Any restrictions set up on your watch, is a profile installed, is it a managed device, or has the Family setup been used for the watch?
I can choose the complications for Heart Rate for my Ultra 2 in the Face Gallery for the Modular Watch face.
Can you select the Heart Rate complication for other Watch faces?
Yes, that is what is shown in the picture, the list of available complications after selecting which complication position to change. The problem is that the heart rate complication is not available to be added to the modular watch face.
Yes. The heart rate complication is available and works on other watch faces I downloaded.
It could only be, that for the Series 9 and the Modular Watch Face, adding the Heart Rate as a complication is not an option.
