Why are AirPods 4 causing bleeding ears?

Stop using them and go see your doctor.
And take the AirPods with you to the doctor.
Is it both ears?
It may be an allergic reaction to the material in the ear tips. Try a different size ear tip so see if it's widespread throughout the tips. Also photograph the bloody ear(s) (not the British bloody mind you) to document the issue.
How long have you had the AirPods? If you're within the 14 day window since purchase you can take them back for a full refund no questions asked.

In regards to an allergic reaction do you have an allergy to silicone?
AirPods 4 don’t have ear tips.
They also don’t go inside your ear canal, they are supposed to sit outside it.
