Apple Watch inconsistently plays iTunes to hearing aids

I have an apple watch 11 gps & cellular but using gps mode only. I'm having trouble playing iTunes to my hearing aids. Mostly goes to watch but sometimes to my hearing aids while at home and while out walking but not consistent.

How can I do this consistently?
