Using iPhone 13 Pro and Mac mini M4. Running latest version of MacOS 26 and iOS 26.
Both devices are meeting all of the requirements, with all of the above turned on, and yet none of the above are working. Please fix as I need them to be working, *particularly* the iPhone being my webcam.

Hmmm...
You can try a Safeboot and see if anything changes(?)
A SafeBoot Start up your Mac in safe mode - Apple Support
Login and test. Reboot as normal and test. Caches get rebuilt automatically.
For sync issue between devices—
Sign out of your Apple ID/Account /iCloud on the Mac and back in again and compare your results
