iOS Contacts app incorrectly

I really hope someone can help me with this. This is convoluted so please be patient and read the entire post. This is issue is new, having only occurred since the 26.1 update. I am a therapist and that's why this is so important because it calls into question HIPAA issues and potential violations. I will be using fake names due to HIPAA. My contacts are set to use the first name for alphabetical listing purposes. I have a client whose name is "Amanda Smith" and a therapist friend whose name is "Amanda Grant". I texted my friend, "Amanda Grant", but the text went to my client "Amanda Smith". I thought I had fat-fingered the contacts and it was my fault. However, the same thing happened vice versa when I texted my client "Amanda Smith" - the text went instead to my friend (HIPAA problem). So, I tried calling....same thing again both ways. You can see the HIPAA issue posed here. I went into my contacts and pulled up my client "Amanda Smith" and her info was correct. I then went into my contacts and pulled up my friend "Amanda Grant" and when I opened her contact card, it read "Amanda Smith" (my client's name) and it had my client's info instead of my friend's info. I tried deleting both contacts and when I tried to delete my friend, "Amanda Grace", it advised me it was a read only and could not be deleted so I edited the info back to my friend's information using "Aman Grace" (dropped the "da" on Amanda). I then added my client, "Amanda Smith", back to my contacts. After doing so, my friend's contact reverted back to the same as my client's info. Since my friend's contact was read only, I added her as a new contact as "Grace Aman" with her correct information. This seemed to work until just today. And guess what??? It has now gone back to what I just explained above. The only reason I was alerted to it was because I got a notification in my contacts that I had a duplicate contact. What the ****? Please help!!!!
