Teaching voice control how to say a word always results in "unable to complete recording"

Voice control always makes the same dictation mistake on my Mac studio OS 26.1 with a few particular words. I've added those words to the custom vocabulary, and then I tried to record how to say the words, but always receive the error message "unable to complete recording". My only option is to either save without recording, or previous. Any ideas?

verify your software is up to date—

The current stable release of Tahoe including bug fixes, security updates is macOS 26.2
