iPad disabled, don't have Apple ID credentials

My dad's iPad was disabled because he tried to log in too many times with incorrect passwords. I was able to reset the iPad, but it's still associated with his Apple ID. We can't log-in or disconnect his account from the iPad because he doesn't the password. We can reset the password because he also doesn't remember the phone number associated with account.


What do we do?

If necessary, if he is the original owner, have him take the device, the original purchase receipt with the serial number, and a photo ID to the nearest Apple Store.
