What does Apple suggest to do if I bought an iphone with apple care in UK but now I moved to Portugal and what if it breaks and Portugal is a country where there are no official Apple Store to fix it. What should I do ?

It's because iPhones sold in different areas of the world use different hardware, particularly for cellular radios. Replacement stock available at an Apple Store outside of your region would not necessarily be compatible or be a direct replacement for your model of phone.
My understanding is that the EU is generally considered as if it were a single country for warranty and AppleCare service. At least while the UK is still legally in the EU. I don't even know if it would matter as long as the purchase was when the UK was still an EU member.
See if you can find an Apple Authorized Service Provider: https://locate.apple.com
Otherwise you would have to take it to a supported country.
Well I could take it to Spain I guess but that would be in vain because as I read from other users complains Apple Care does only work in the country where it was purchased.
