Setting up Apple ID on old iPhone 4.

Hi trying to set up a new apple is for my daughter on iPhone 4...but keeps on asking me to enter verification but there isn't anywhere to put it...can anyone help?

If you see a password box, enter the password (ABC) followed by the code (123) or ABC123 (no spaces).
