That setting is only in effect when Do Not Disturb is turned on; if Do Not Disturb is off, it doesn't matter whether that setting is on or whether Always is selected.

Some other things to check:

The ring/silent switch on the left side of the phone. See: How to put your iPhone on vibrate, ring, or silent mode - Apple Support

Settings > Phone > Silence Unknown Callers: Detect and block spam phone calls - Apple Support

There is no silence unknown callers in this phone. The software is IOS 12.4.8. The phone does ring. It gives an incorrect ring tone. I tried changing the ring tone and rebooting the phone. Still does not ring correctly.

Thanks have tried all that, still have a problem. I think it is hardware related.
