Bluetooth was not on. After turning it on my TV appeared in the Airplay list and the Airplay icon appeared in the menu bar.

Are you sure that wifi and bluetooth are both on, and that both your mac and your target airplay device are on the same wifi network?
How can I turn on Airplay if the Airplay icon is not in the menu bar?
