4-5 times a month for the last couple years my Apple ID gets locked.

My understanding is it locks when someone fails to login too many times in a row. This isn't me. How can I find out info on who is trying to login to my account? Even if it's just an IP address it would be a start.

Immediately secure your iCloud by changing the password. If anyone tries to login with your credentials then you’ll definitely receive a notification about that attempted login with device and location details. Can you confirm if you’re receiving those notifications?
