Why this Occurred?:
As for why this occurred without your knowledge, you should modify your credentials immediately, starting with a new, strong password and using a new eMail address (for Apple). This is because someone may have your credentials, though a full assumption, on my part.
So, log into iCloud, and modify your credentials,
- Modify: your login credentials (Using Both a New eMail and a New Passwordfor each and all sites)
- Associate: a different method of payment to your account
- Enable: Two-Factor Authentication for Apple ID and for all other accounts that have such an applicable option
