focus letting notifications through

last night my wife's iphone let seveal notifications through (screen on and tri tone), even though she had focus turned on and no apps or people allowed. Shen has an iphone 11 and 15.2.1 ios.

Check whatever app notified and turn off Time Sensitive. I can't see how else they got through.
