Monterey 12.1 Update causing "(!)"

Does anyone know why Monterey 12.1 update causes 16" MacBook Pro M1 to display circle "(!)"? My computer had this problem which was fixed with "Revive." Is this caused by computer not being connected to power during update or some other random issue? Apple store said 1 of 12 problems like this are fixed with "Revive" and 11 of 12 need to be "Restored" with HDD reformat (total data loss). Seems like a serious issue which should be reported to buyers in a disclaimer warning (e.g., updating Monterey may cause iOS failure. Recommend backing-up HDD before updating or else?). Apple should have a webpage for users to report issues with this problem.

I am aware of instructions at About macOS Recovery on Intel-based Mac computers - Apple Support (CA) . These instructions are correct, but key instructions won't work to solve this problem (e.g., turn on Mac and immediately press and hold Command (⌘) and R and release keys when Apple logo and spinning globe appears). Users can try this or use another computer with USB-C male-male data cable and "Revive" software to solve the problem, but most users won't have another computer or the USB-C male-male data cable. The problem can also be resolved by taking the computer to the apple store where they have "Revive" software and another computer with USB-C male-male cable to solve problem. Apple informed me that only 1of 12 computers with this issue are corrected with "Revive" software. Most computers with this issue need to be restored with loss of HDD data.
