if I buy a movie in Germany can I watch the movie in English ?
I want to buy a movie in Apple TV but I want to be sure I an watch it in English with Spanish subtitles even though I am currently in Germany

It depends on the region for your Apple ID account. If you have your account billing address set to Germany, then you would be shopping in the German iTunes Store, with the German movie catalog, which may or may not have the audio languages and subtitle languages that you seek.
Supplying studios can determine for each title and for each region what language support to include. Some movies may include English audio and Spanish subtitles, but others may not. Spanish subtitles will be rare. The Details page for each movie will list which audio languages and subtitle languages are included, so that you may check that before making a buy or rent decision.
E.g. The Batman in Germany has German and English audio, and 35 subtitles tracks.
E.g. Uncharted has German and English audio, and 1 subtitle track (German).
Modern Warner Bros movies seem to be generous with language support, others less so.
