There is a world of difference between those two reports. In the first one in March, your hard drive is practically dead. The one from today is within expectations for that Fusion configuration. THe File System scores, an indicator of drive health, is horrible ("timed out"), but in today's showing excellent health.
What happened between the two tests?

Predicated upon an earlier posting regarding 1 or both of the Internal Drives are the root cash of all the issues
Save yourself a trip to an Certificated Apple Service Center
Purchase an External Enclosure and a matching SSD Drive with at least 1 - 2 TB Capacity
Good place to start might be www.owc.com
Install the Operating System to start up the External Drive
Set the computer to run from the External Drive
All this may extend the life of the computer as well as doing all this yourself
Is the RAM installed original, or did you upgrade it at some point.
My first guess would be incompatible, marginal, or bad RAM.
I would still like to know if the RAM is original or aftermarket.
I've seen too many bizarre crashes with aftermarket RAM to dismiss that as a possibility.
Failing hard drive - This computer has a hard drive that appears to be failing.
The KP implicates that suspicion. It should not have been difficult for your Local Mac support to have reached that conclusion quickly.
That Mac's startup drive performance is still abysmal. It needs to be replaced.
Memory failures do not manifest in poor performance. The Mac will either fail to start, or it will crash to a black screen.

I have installed aftermarket RAM, but its been working fine for more then two years and now its getting worse and worse
Apple support didn't find any thing wrong at the shop?
MS Software? Is there a problem with Office 365?
I try to start the computer with different RAM installed, in the one attached the computer has been shut down for days and now started with one RAM of 16m installed inport no 3.
