All of the information the error asks for are already entered in the form? I've put my phone number area code, city zip, etc... yet the entry fields are ringed with red as though I didn't fill them in? I've tried multiple times but can't seem to get it to accept any information.

There is only so much we can or should do on this site, because further discourse may require that you reveal personal information. Don't do that. But do "sign in" to your Apple Account, here: https://account.apple.com/, and confirm everything there is correct.
For posterity: I eventually figured it out, kind of. Logging in online and logging in through the settings menu before logging into the app store fixed the issue.
I must have been composing my reply when you added that information. I think you're on the right track. Give it a few minutes for the information provided in https://account.apple.com/ to populate the phone numbers in the App Store.
Consider restarting your Mac in the usual manner. It might help accelerate that process.
Some progress: adding my address on account.apple.com auto-filled it into the app-store form, and now I am just getting:

It's still not autofilling the phone # after restarting (& still does not accept when I enter manually), I'll give it a few hours and check again.
