"Album by artist" has suddenly appeared as the first column in my smart playlist when it wasn't there before and clicking view options there is no box to get rid of it. How could this have happened and how can I remove the column?

Click it.
As you click, it will cycle thru "Album," "Album by Artist," etc. Find one you like, or you can get rid of the whole column by using View Options.
Edit: By the way, in View Options, it is always called "Album" no matter what you have currently toggled it to.

News! - I tried closing and opening the playlist and 'Music' application aborted. When I restarted the same smart playlist was back to normal, so another bug that gets fixed by a restart.

Hi thanks - I knew the first bit and don't want any of them - the second sentence did not sync with reality - in view options there is no box to check/uncheck "Album by..." there is a checkbox for "Album" which I have checked and that's a whole separate column.
