How do you get live captions feature to work when your phone is connected to your cochlear implant?

When live caption is turned on and it’s set to the iPhone audio and my iPhone is connected to my cochlear implants, the live captioning doesn’t work.

iOS 26?

I was noticing that since iOS 26 I am not getting key-clicks in my hearing aids, and I thought I had just messed up some setting or other.
