iTunes: Update music library paths after moving to new PC

I'm trying move iTunes to a new PC. Here is the issue I'm running into. My old PC doesn't have a very large capacity hard drive so I had to use a flash drive that holds all my music files. So, the program itself, my playlists, play counts etc., are stored on the C drive of the PC. My music folder is on the D drive. I was able to move the iTunes folder from old C drive to new C drive and everything was there with no issue. When I went to bring the music folder over from the D drive, all the files point to the D drive on my new PC which isn't there. Is there a way to change the path of the entire folder at once? I've had to locate individual files on occasion which is no big deal but if I have to do that with the entire folder that would be a major undertaking. Any help is appreciated.
