MacBook crashes

My MacBook crashes and attempts to restart repeatedly without success. It will come up in Safe mode and runs fine for several weeks but then it goes to a black screen, crashes and reboots again. The EtreCheck report is attached. Does anyone have any ideas on what's causing this? Thanks

The 15" model is known to have GPU issues. You can try forcing the laptop to use the Intel GPU instead by using gfxCardStatus:
