Iphone 5 with cell or not?
do I need to buy the apple watch 5 with cellular or is this just creating another fee for the carriers if I have an iphone that will be with me?

As mentioned, the GPS only Apple Watch can do Everything the cellular model can do as long as your iPhone is nearby, where nearby is say in the same 3-4 bedroom sized home.
The cellular model allows you to leave your iPhone at home, while you go for a walk, run, bike ride, etc... and still make/receive phone calls, send/receive messages, etc...
The Cellular model costs more than the GPS only model (approx $100 extra more or less).
If you get a Cellular model, AND you decide to activate the cellular radio, then a carrier that supports the Apple Watch will charge you a monthly fee to share your iPhone phone number with your Apple Watch. In the U.S. the fee is $10 per month.
