iPhone Do Not Disturb issue

Do Not Disturb is not turned on, yet the "silence when locked" box remains checked. I am missing calls.

If DND is turned off, the fact that that box is checked is irrelevant. It only has an effect if DND is enabled. So, that's not why you're not getting calls.

Other things to check:

Is the mute switch enabled? It's the button on the side above the volume buttons,
Is the volume turned down?
Do you have Settings>Phone>Silence Unknown Callers enabled?
